Hollow-looking hands: what to review before filler

Hollow-looking hands or visible veins and tendons do not automatically mean that volume should be added. First review skin thickness and movement, side-to-side differences, daily hand use, previous procedures, and medical history.

Review volume and skin thickness separately

Structures beneath the skin are close to the surface on the back of the hand, so veins and tendons may look more prominent when skin is thin or volume has decreased.

We compare the hand at rest with a gentle fist and distinguish surface dryness from a change in contour.

Record vein and tendon position and side-to-side differences

Note whether one side is more prominent, whether movement changes the contour, and whether swelling, pain, or skin-color change is present.

Photographing both hands together under the same light helps separate angle-related differences from persistent findings.

Include hand use and previous history

Tell the clinician about your dominant hand, sports, instrument or tool use, recent injuries, and responses to previous injections or procedures.

Current medicines, bleeding tendency, allergies, inflammation, and open skin on the hand are also important.

Choose product and injection sites after an examination

Approved areas and product characteristics differ, and fillers carry risks ranging from bruising and swelling to rare but serious complications.
