No Botox Results Yet: Should You Get More Right Away?

  • Botox Injection

A lack of immediate change after Botox for expression lines is not, by itself, a reason to get more injections right away. The American Academy of Dermatology says most people see results within 3-7 days. That window is not a deadline for everyone’s final result or a recommended date for additional injections.

Hello, I am Dr. Kim Yeon-jin, Chief Physician at UH CELL Clinic Gangnam.

I want to distinguish when changes become visible from when to consider another injection. This guidance concerns expression lines, such as frown lines, forehead lines, and crow’s feet. Do not use it to set treatment schedules for jaw muscles, body treatments, or excessive sweating.

Does no immediate change mean Botox has not worked?

According to the American Academy of Dermatology’s overview of botulinum toxin therapy, most people see results within 3-7 days after treatment. Not noticing a difference in the mirror immediately after an injection does not establish that the treatment has failed.

I want to emphasize what that timeframe means. It describes when changes generally become visible. It does not mean that everyone can have additional injections after day seven. Assessing the current response and deciding whether to inject more are separate decisions.

An insufficient initial response is different from returning wrinkles

The American Academy of Dermatology explains that improvement in wrinkles lasts about 3-4 months, sometimes longer. Its treatment FAQ discusses another treatment when wrinkles reappear.

I recommend distinguishing these two situations when you describe your concern:

  • You feel the change has been insufficient from the start: Discuss how long it has been since treatment, what change you expected, and what your current condition is.
  • You noticed a change, but the wrinkles returned over time: This is the situation the Academy describes when discussing another treatment. Even so, a general duration of effect does not determine your individual retreatment date.

Do not interpret the 3-4 month duration as a schedule for an early touch-up. It is also not a recommendation to get additional injections on a fixed date simply because you feel the response is insufficient.

Prepare your previous injection details before discussing more treatment

The 2025 revision of the onabotulinumtoxinA guidance on MedlinePlus, from the U.S. National Library of Medicine, advises telling your clinician if you have received any botulinum toxin injections within the previous 4 months. Your medications and any previous adverse reactions are also important information to share.

I recommend writing down whatever you know about your previous treatment: the date, area treated, product name, and dose. Do not guess at details you do not know.

The 4-month period here is a treatment-history disclosure precaution in the guidance for this particular medication. It is not a universal interval for additional injections across all products.

Some symptoms need medical attention, not more time

The same MedlinePlus guidance advises contacting your clinician if bruising, swelling, or pain at the injection site is severe or does not go away.

It also lists new eyelid drooping, double vision, or blurred vision as reasons to contact a clinician immediately or seek emergency treatment. Seek immediate medical help if you have difficulty breathing, swallowing, or speaking, or if you develop muscle weakness throughout your body.

I urge you not to treat these symptoms as a delayed cosmetic response and simply wait for results.

If you have an upcoming departure, ask when to have your response assessed

MedlinePlus advises asking your clinician when improvement should be expected and contacting them if you do not improve within that period. If you have an upcoming departure, I recommend preparing these questions:

  • When should I expect to notice changes from my treatment?
  • If there is no improvement by then, when should I seek a clinical assessment?
  • What should I do if concerning symptoms develop?

Rather than setting a date for additional injections just because your departure is approaching, bring your previous treatment details and discuss your current response and follow-up timing with your clinician.
