Are all the small dots on your nose blackheads?

Hello, I’m Dr. Kim Yeon-jin, representative director of UHCELL Clinic Gangnam.

Seeing dots return after a pore strip can feel like an incomplete clean-out. But not every dot on the nose is a blackhead that needs removing. I check for a blocked comedone versus a visible normal oil channel before considering extraction or a pore procedure.

A blackhead is not the same as a sebaceous filament

A blackhead is an open comedone: oil and dead skin material plug the follicle opening. Its dark appearance does not mean poor washing; exposed material can darken in air.

Sebaceous filaments are fine structures within the channels that carry oil to the skin surface. They can appear as small, fairly uniform gray or pale dots on the nose. They are part of normal skin, not something that must all be removed.

Appearance alone cannot confirm a diagnosis. A single dark spot that keeps growing or bleeds needs a separate assessment rather than being treated as a pore.

Repeated squeezing can create a different problem

Fingernails, metal tools, and repeated pore strips can cause redness and injury. Inflammation or a lingering mark may become more noticeable than the original dot.

I prefer reducing unnecessary irritation to trying to empty every pore at once. Normal oil structures can become visible again, and blackheads need ongoing care to reduce new comedones.

If an extracted area stings or peels, do not add scrubs, acids, or more squeezing. Spreading redness, increasing pain, or pus warrants medical review.

Check for overlapping exfoliants first

Ingredients such as salicylic acid can help with blocked pores but may also dry or irritate skin. Note any retinoid, other exfoliant, or recent peel. More is not automatically better.

  • Where the dots are, and whether any area is red or painful
  • When you used strips, scrubs, or extraction
  • Names of cleansers, acids, and retinoid products
  • Changes in stinging, peeling, or redness afterward

Starting several products together makes the irritant harder to identify. Do not stop prescribed medication on your own; ask about its use if irritation occurs. Keep cleansing gentle and use moisturizer rather than scrubbing harder.

A consultation is not about erasing every pore

Pore size, oil, comedones, and surrounding skin firmness are different concerns. Small dots alone are not a reason to select a strong laser. Existing irritation matters too.

I start with what you want to change: blackheads, greasiness, or visible pores. Each gives us a different goal and helps separate something worth treating from a normal structure.

Normal pores and oil channels cannot be permanently eliminated. Suitability and recovery vary. Discuss what is realistic for your skin rather than using a perfectly smooth edited photograph as the target.

Frequently Asked Questions (FAQ)

Do returning dots mean my care has failed?

Not necessarily. Normal sebaceous filaments can become visible again. First establish whether the dots are comedones or normal structures.

Should I use pore strips more often?

Increasing use can add irritation and injury. Check the skin first, and avoid extra irritation on stinging or peeling areas.

Should I book a pore laser straight away?

Review comedones, oil, irritation, and your goal first. Not every dot is a laser-treatment target.

At UHCELL Clinic Gangnam, bring a record of when the dots appeared and the products or extraction methods used. I consider the distinction between treatable comedones and normal structures before adding a procedure.
