When the jawline looks wide: Why masseter Botox consultation should also check jaw discomfort

Hello, I am Dr. Kim Yeon-jin, Chief Physician at UHCELL Clinic Gangnam.
A wide-looking jawline does not have one universal cause. Facial structure, masseter thickness, fat distribution, skin laxity, and camera angle can overlap.
During a Botox consultation, I compare the masseter at rest and while clenching, review left-right differences, and ask separately about pain or limited mouth opening.

Compare the jawline at rest and while clenching
The masseter is one of the main muscles used for chewing. Checking whether the lower jaw becomes firm and prominent during clenching, and whether one side is used more, helps clarify how much muscle contributes to the contour.
One photograph or jaw width alone cannot identify the cause. Bone structure, fat distribution, and skin laxity should also be reviewed before discussing treatment.
Separate jaw pain and limited opening from cosmetic goals
Pain while chewing, limited opening, locking, or painful clicking may require evaluation of the temporomandibular joint and surrounding tissues first. Joint sounds without pain are common and do not always require treatment.
Evidence is not conclusive for using botulinum toxin as a general treatment for temporomandibular disorders. A cosmetic masseter Botox consultation should not replace diagnosis of jaw pain; dental or related evaluation may be recommended.
Set realistic contour goals and recovery timing
A consultation should review the desired contour, previous Botox response, changes in chewing strength, medication, and important plans. Injection sites and dose are determined after examining facial proportions and muscle activity.
Temporary redness, tenderness, or bruising may occur. Sudden facial asymmetry, weakness on one side, or speech difficulty requires urgent medical evaluation rather than waiting for a cosmetic appointment.
Frequently Asked Questions (FAQ)
Is every square-looking jaw caused by large masseters?
No. Bone structure, muscle, fat distribution, and skin laxity can all influence the jawline and should be assessed separately.
Can I have masseter Botox if my jaw clicks?
Clicking alone is not enough to decide. Pain, limited opening, locking, or a bite change may require TMJ evaluation first.
What should I share before consultation?
Bring details about previous Botox, jaw pain or locking, medication, your desired change, and important upcoming plans.
A wide jawline is not explained by the masseter alone. Separate muscle activity, facial structure, fat distribution, and jaw symptoms before choosing treatment.
