
One cheek feels hotter during Thermage? Tell your clinician

If Thermage feels noticeably hotter as the handpiece moves from your cheek toward your cheekbone, should you try to tolerate it? Speak up first. A change in where or how strongly you feel heat gives your clinician information they need to reassess treatment.
I don’t use a person’s ability to endure pain as a measure of results. I think of treatment as finding the range of thermal stimulation their skin can tolerate. More pain on one side does not show that energy reached that side more appropriately.
Why your cheek and cheekbone may feel different
Thickness varies. Skin thickness, subcutaneous fat distribution, and sensitivity differ across the face. Thin, sensitive areas around the eyes, temples, and cheekbones can feel different from the cheeks or jawline. A left-to-right difference in sensation cannot diagnose what is happening beneath the skin.
Thermage uses monopolar radiofrequency to deliver thermal stimulation to the dermis, the layer containing collagen that supports the skin. Brief heat from the tip comes with cooling and vibration. Cooling does not make every area feel the same or eliminate the risk of burns.
A clinical principle: check contact and skin response
Endurance isn’t the goal. I prioritize pulse placement that suits the facial structure and even tip contact over the level displayed on the screen. Excessive pain can make someone tense their face or change position, making uniform contact harder to maintain.
Telling the clinician where it feels uncomfortable helps them reassess skin response, contact, and the treatment area. They decide whether to adjust the energy or pause. Asking for less pain does not mean treatment will stop working. Equally, feeling little pain is not proof that treatment was adequate.
What to say during treatment
Name the spot. Explain where you feel it, what it feels like, and how long it lasts. These are examples of ways to describe sensations, not quotations from actual patients.
- My right cheekbone feels much hotter than my cheek did a moment ago.
- One spot feels sharp, and it still stings after the tip lifts away.
- I’m tensing my face and would like a short pause.
- The area suddenly feels less sensitive.
Speak before moving. Let the clinician stop and check rather than changing your position or cooling the area yourself. If asked for a pain score, describe your own sensation at that moment instead of comparing yourself with other people.
What to disclose before treatment and watch for afterward
History matters. Mention areas that hurt unusually during previous lifting treatments, prolonged swelling, the location and timing of fillers, medicines, and medical conditions. People with a pacemaker or another electrical implant must not undergo Thermage. Pregnancy, breastfeeding, and areas with altered sensation also need discussion before treatment.
Redness or swelling can occur afterward. Less common reported effects include burns, blisters, skin surface irregularities, and changes in sensation. Contact the treating clinic if blisters develop or pain worsens, rather than enduring it at home. The amount of pain during the procedure cannot establish how well the skin is recovering.
Frequently Asked Questions (FAQ)
Does a hotter feeling on one side mean different results?
Heat sensation alone cannot compare results between sides. Skin thickness, sensitivity, and contact need assessment together. Tell your clinician the location as soon as you notice a difference.
Will asking for less pain reduce the results?
Pain intensity is not a measure of results. Energy and pulse placement should be assessed against the skin’s response rather than simply increased.
If my skin is thin, is a lower level all I need?
Lowering a number does not resolve every concern. Hollowing, fat distribution, and areas to treat or avoid need assessment first.
Can I just wait at home if my skin stings afterward?
Contact the clinic about new blisters, worsening pain, or persistent altered sensation. Recording when the symptom started and its location helps you explain what has changed.
