Focused ultrasound targets a set depth rather than skin pigment, and a published study found no significant change in epidermal barrier measurements after treatment. Redness, swelling, muscle soreness and occasional altered sensation are documented reactions, so current skin condition, the area that needs lifting and your travel schedule still need to be reviewed before planning it.

Hello, I am Dr. Kim Yeon-jin, Chief Physician at UHCELL Clinic Gangnam. Patients often ask me, “My skin is too sensitive for lasers, so is lifting off the table too?” I will not repeat laser concerns here. Instead, I want to walk through how we actually evaluate HIFU lifting, such as the Shurink Universe program we run at our Gangnam clinic, for sensitive skin.

HIFU and lasers deliver heat to different places

The main worry patients raise about lasers on sensitive skin is surface reaction and pigment change. Mayo Clinic notes that skin can become darker or lighter after laser resurfacing, and that the risk of long-term color change is higher in brown or Black skin. The darkening is called post-inflammatory hyperpigmentation.

HIFU works differently. As we describe on our Shurink Universe program page, this device concentrates high-intensity focused ultrasound energy at a target layer, mainly the SMAS fascia, to create thermal coagulation points and stimulate collagen. The heat is delivered as discrete points at a chosen depth inside the skin, not across the surface.

That distinction matters for sensitive skin for two reasons.

Pigment exposure: a review in the dermatology journal Cosmoderma reported that microfocused ultrasound with visualization (MFU-V) bypasses the melanin-rich epidermis, which lowers the risk of post-inflammatory hyperpigmentation, and that clinical trials and expert consensus rated its safety profile favorably, including in patients with Fitzpatrick skin types III-VI.

Barrier function: a study published in Clinical, Cosmetic and Investigational Dermatology found that after a single MFU-V (Ulthera system) treatment, transepidermal water loss, skin hydration and erythema showed no statistically significant difference from baseline through 24 weeks. The researchers concluded the treatment did not alter epidermal barrier function or skin physiology.

One qualification is worth stating plainly. Those studies were mainly run on Ulthera-type devices, and results do not automatically transfer across different devices and settings. That is why I explain HIFU to patients worried about surface irritation as an option with supporting evidence to review, and I decide whether to proceed only after an in-person diagnosis.

What you may actually notice after treatment

HIFU still carries post-treatment reactions. Here is what the published research reports, organized by reaction type.

Reaction

What the research reported

Source

Redness and swelling

Mild erythema and edema appeared in all participants and lasted 2-48 hours after treatment

Dermatology and Therapy (2023), randomized controlled trial of 20 participants

Tenderness to touch (allodynia)

Treatment-area sensitivity with no visible change was common and generally lasted around 14 days

Same study

Muscle soreness

Reported in 90% of participants, with a median duration of 6 days before resolving on its own

Lasers in Medical Science (2026), prospective study of the mid and lower face

Uncommon reactions

Altered sensation (numbness or hypersensitivity), bruising, tingling, burns, linear marks and contact dermatitis reported in roughly 2% of cases

Systematic review of microfocused ultrasound published on PMC (U.S. National Library of Medicine)

The reaction most relevant to sensitive skin is allodynia. Your skin can look completely normal while still feeling sensitive when you cleanse or touch it. Since the research places this at around two weeks, knowing about it ahead of time should make it less alarming if it happens.

The same systematic review listed severe skin laxity and a body mass index above 30 as relative contraindications, since outcomes were reduced in those cases. It concluded that microfocused ultrasound is effective at tightening mild to moderately lax facial skin in women. That is why we check both how sensitive your skin is and how much laxity is present during consultation.

What we adjust in the plan for sensitive skin

The same device produces a different treatment depending on which depth receives energy and how much. As we explained in our post on Shurink Universe pricing differences, a patient with more buccal fat, thinner skin, jawline concerns or perioral laxity each needs a different depth and range of energy.

The Dermatology and Therapy (2023) study explains that a 4.5mm focal depth mainly affects the SMAS layer and deep fat for contour lifting, while a 3.0mm focal depth mainly affects the superficial fat layer and fascia for tightening. For sensitive skin, I look at three things together.

Current surface condition: whether there is active rosacea, seborrheic dermatitis, atopic dermatitis or acne right now.

Where the laxity starts: whether the main concern is deep-layer laxity or superficial loss of elasticity, which determines how depth is allocated.

Pain tolerance: if you are sensitive to pain, raise anesthesia options during consultation first. Applying numbing cream over a wide area needs its own precautions, which we cover separately in our numbing-cream safety guidance.

Planning this around a short stay in Seoul

Under our Shurink Universe program guidelines, treatment takes about 40 minutes, patients can resume daily activity right after, and the maintenance period is guided at around 3 months, with individual variation. The relatively light downtime is one reason patients visiting Seoul for 2-7 days find it workable to consider.

When you build it into a travel schedule, factor in the recovery times from the table above. Redness and swelling usually settled within two days in the research, but muscle soreness or allodynia can continue after you fly home. The same applies to judging results: the Clinical, Cosmetic and Investigational Dermatology study found skin elasticity increased significantly at 12 and 24 weeks after a single treatment, so it is more accurate to expect the result to continue developing for months after you return.

Our Gangnam location is inside the UH FLAT SIGNATURE Gangnam hotel, so the treatment and recovery routine can continue inside where you are staying. Japanese, Chinese and English-speaking staff are on site at the Gangnam clinic.

Tell us this before your consultation

For sensitive skin, the information you bring in advance changes how we design the plan. Photos or notes covering the following are enough.

Diagnosed skin conditions and current medication: let us know about any history of seborrheic dermatitis, atopic dermatitis or rosacea, and any topical or oral medication, including steroids or isotretinoin.

Recent procedures and dates: tell us when you last had laser treatment, chemical peels, Botox or filler. The Cosmoderma review cites research recommending at least a 14-day gap between botulinum toxin injection and MFU-V treatment.

Past reactions to procedures: if redness lasted unusually long, or you have had pigmentation changes or blistering before, tell us which procedure caused it.

Bruising tendency and allergies: let us know if you bruise easily or have known drug or product allergies.

Return date and itinerary: what else you plan to do on treatment day depends on how many days remain in your stay.

Contact us right away if you notice

Research also reports occasional blistering, erosion or ulceration, tissue atrophy and nerve injury, and notes that improper device handling or settings that deliver energy too superficially can lead to these complications (Dermatology and Therapy, 2023). Do not wait to contact us if you notice any of the following.

Blistering or weeping skin

A clearly visible linear red mark

Uneven movement on one side of the face, or a corner of the mouth or eyebrow that will not move normally

Numbness or tingling that has not improved after several days

If you have already returned home, send us photos of the treated area and also see a local physician.

FAQ

Is HIFU a type of laser?

No. HIFU uses focused ultrasound, a different energy source from laser, which uses light. That is why the pigment reaction people worry about with lasers and the reactions to watch for with HIFU, such as muscle soreness, allodynia and altered sensation, need to be considered separately.

Can I get HIFU if I have rosacea?

Having rosacea alone does not determine eligibility. In the Dermatology and Therapy (2023) study, redness immediately after treatment appeared in all participants and resolved within 2-48 hours. Send photos of your current rosacea and any recent flare-ups, and we will set the timing and scope after diagnosis.

Does sensitive skin mean more pain during treatment?

Pain depends on the device, depth and treated area. In the Dermatology and Therapy (2023) study, the median pain score during treatment was 3 out of 10, and participants did not need separate pain medication before or after. If pain is a concern for you, raise it during consultation first.

If I cannot return to Korea easily, is one treatment enough?

The Clinical, Cosmetic and Investigational Dermatology study observed increased elasticity at 12 and 24 weeks after a single treatment. Our guided maintenance period is around 3 months, with individual variation. How soon to schedule a follow-up should be decided during consultation based on your degree of laxity and how you responded this time.

Next step

If sensitive skin has kept you from considering lifting treatment, start by preparing the items listed under “Tell us this before your consultation.” Once we review your skin condition, the areas of laxity and your travel schedule together, we can determine during diagnosis whether HIFU is appropriate and how to allocate depth and range.

※ Skin reactions and recovery periods vary by individual.
