Key takeaway: During or immediately after filler injection, unusual pain, white, gray, or blue skin near the injection site, or vision changes need immediate care
Hello, I am Dr. Kim Yeon-jin, Chief Physician at UHCELL Clinic Gangnam.
If you develop any of these symptoms during or immediately after a filler procedure, I recommend seeking medical attention immediately rather than assuming it is bruising. The U.S. Food and Drug Administration (FDA) identifies these as warning signs. They do not all have to occur together.
My advice is not to let an explanation of common bruising and swelling become a reason to dismiss a warning sign. That explanation does not establish that your current symptoms are normal.
Common bruising and swelling do not make every kind of pain normal
The FDA’s consumer safety guidance lists bruising, redness, swelling, pain, and tenderness among common reactions to dermal fillers.
I would not want you to interpret “filler can be painful” as a reason to tolerate unusual pain. The FDA’s patient guidance separately advises seeking immediate medical attention for unusual pain during or immediately after the procedure.
White, gray, or blue skin near the injection site requires attention
According to the FDA’s consumer safety guidance, filler unintentionally injected into a blood vessel can cause serious complications, including skin necrosis, stroke, or blindness. The FDA describes the likelihood as low, but the damage can be permanent. Skin necrosis means that skin tissue is damaged and dies.
If skin near the injection site looks white, gray, or blue during or immediately after the procedure, I recommend seeking medical attention immediately. The FDA identifies these specific color changes as warning signs. Do not wait for pain to develop as well.
Watch for vision, speech, and movement changes during or immediately after injection
The FDA’s patient guidance calls for immediate medical attention if vision changes or stroke symptoms occur during or immediately after the procedure. The listed symptoms include sudden difficulty speaking, numbness or weakness in the face, arms, or legs, and difficulty walking.
If these symptoms occur during or immediately after your procedure, I recommend seeking emergency medical care immediately rather than waiting for your next appointment or a reply to an inquiry. Report changes in your vision, speech, or limb movement, not just changes in the appearance of your skin.
What to tell the medical team and discuss before another procedure
When seeking care, tell the medical team when and where you received filler, the product name if you know it, when your symptoms began, and how they have changed. You can show treatment records or photos you already have. Do not delay care to find records or take pictures.
Before considering another filler procedure, ask your clinician to explain the patient information for the product and the warning signs to watch for. The FDA also recommends reading the product-specific patient information and discussing it with your clinician.
I encourage you to understand not just the changes you hope to achieve, but also which symptoms require immediate medical attention before deciding on treatment. If any of these warning signs develop during or immediately after your procedure, seek care immediately.
